The topic of handling medical records really changed when CMS mandated all medical records become an EHR (electronic health record). When there was a such thing as paper medical charts there were many precautions, I would take being a medical professional. The first one I would take is to make sure that I am documenting in the right chart. When documenting in paper charts it was easier to make the mistake of writing in the wrong chart and when you are handwriting you can not hit the backspace button and we all know we cannot white something out in a medical chart. So, making sure I had the right chart, right patient and all three identifiers was on example of the precautions I would take when handling a medical record.
